What Is Breast Revision Surgery?

Breast revision surgery, also called secondary breast surgery, is a procedure that corrects or improves the results of an original breast augmentation or lift. It is usually performed when the patient is dissatisfied with the outcomes of their first operation. Breast revision usually takes 1-3 hours, depending on the complexity and exact procedures.

Goals of Breast Revision

Women undergo breast revision surgery for many reasons. They may want to improve their appearance or feel more comfortable in their bodies. In most cases, patients aim to achieve one or several of these goals:

Breast Implants Removal
Breast implant removal involves surgically taking out breast implants. The breasts are then lifted and reshaped to create a youthful and firm appearance. Reasons for removing implants include complications, dissatisfaction with the look or feel, or a desire to go back to natural breasts. The surgeon takes out the implants and surrounding scar tissue, closes the incisions, and helps the breasts return to a natural shape.
This surgery is often performed after the initial breast augmentation procedure that did not bring the desired results. Breast implant revision surgery is designed for women who want to change the type, shape, or size of their existing breast implants. Common reasons include complications and dissatisfaction with the current look. Patients may also want to shift from silicone to saline implants or vice versa. By replacing breast implants, our surgical center can help women feel more attractive, comfortable, and youthful.
Breast pocket revision corrects implant pocket problems like asymmetry, deformity, or implant displacement. This plastic surgery modifies the size, shape, or placement of the implant pocket to ensure proper implant positioning and improve breast appearance.
During a fat transfer surgery, Dr. Erella uses liposuction to take fat from different parts of the patient’s body and inject it into the breasts to increase their volume and improve contours. This option is ideal for women seeking subtle or moderate changes (no more than 1 cup size). The transferred fat integrates into the breast tissue, creating a softer, more natural feel than implants.

Breast Lift Revision Surgery

This revision surgery corrects issues from a previous breast lift. Problems like recurrent sagging and uneven breasts may require an additional procedure. Dr. Erella adjusts and tightens breast skin and tissue, repositions nipples/areolae, and improves breast contour, making the patient look more youthful.

Scar Revision Surgery

This operation corrects problematic scarring from earlier procedures, such as breast augmentation, lift, reduction, or reconstruction. In some cases, patients are left with hypertrophic scars, which are raised, thick, and painful. Revision surgery removes or alters these traces to make them less visible.

Breast Revision Procedure

Anesthesia

Breast revision surgery is performed under general anesthesia to ensure patient comfort and safety. The anesthesiologist carefully monitors vital signs and adjusts medications to achieve the appropriate level of unconsciousness throughout the procedure.
Dr. Erella makes incisions to access the breast tissue that needs revision. He may use one of the following patterns:
  • crescent (along the upper border of the areola)
  • donut (around the areola)
  • lollipop (donut + a vertical line to the breast crease)
  • anchor (lollipop + a horizontal incision along the breast crease)
  • inframammary (only along the breast crease)
  • transaxillary (in the armpit)
The incisions are carefully planned to minimize visible scarring while allowing the surgeon to have the best access and visibility.
Dr. Erella reshapes the breast tissue, moves fat from one area to another (in case of fat transfer), re-arranges the internal sutures, and revises the scarring from previous surgery. Excess breast tissue may be removed to reduce sagging. Implants can be replaced, repositioned, or changed in size. In addition, the surgeon may perform a capsulectomy to remove scar tissue. These procedures are tailored to each patient’s aesthetic goals.
Once the breast revision is complete, Dr. Erella closes the incisions with sutures and surgical tape. Dissolvable stitches may be used for more comfortable recovery. The incisions are dressed and bandaged to promote healing after surgery.

Immediate Recovery
Patients can go home the same day after the operation. The first 24 hours should be dedicated to rest, managing symptoms with prescribed medications, and keeping the surgical dressings in place.
During the first week, swelling and bruising reach their peak. The original bandages are removed, and discomfort continues as the breasts heal. Patients must be diligent about taking medications and avoiding strenuous activity.
Most people return to work after 7-10 days of recovery, but this period can be longer if the surgery is complex. Patients must attend all follow-up appointments during weeks 2-3 to ensure proper healing. They should be careful to avoid physical exertion for at least 1 month.
By weeks 4-6, most swelling, bruising, and soreness have improved significantly. While exercise can gradually pick up, patients must still limit strenuous activity to allow the breasts adequate healing time. Full recovery takes about 6 weeks for most people. However, the final results are visible only after 6-12 months.
